What the intake process is like

Time to understand your concerns, hear your child’s perspective, and discuss next steps.

Getting started

  1. Request an initial call

Complete a short online request. We’ll follow up to arrange a brief phone call to hear your concerns and discuss whether an initial evaluation at Blue Horse is appropriate. Families referred by another provider also complete this step.

  2. Prepare for your visit

If we move forward, you’ll receive a link to intake forms and questionnaires to complete through the secure patient portal before your appointment.

  3. A two-part intake

The intake includes a psychiatric evaluation lasting approximately 90 minutes, followed by a treatment-planning visit lasting approximately 45 minutes. This allows time to understand your child’s needs before discussing recommendations together.

Part one: getting to know your child

Before we meet, you’ll complete intake paperwork about your child’s development, medical and mental health history, family background, school experiences, and previous treatment. This gives us a starting point for our conversation.

The initial psychiatric evaluation lasts approximately 90 minutes. We’ll explore what brings you in, hear your concerns and goals, and learn about your child’s strengths, interests, relationships, and everyday life. Together, we’ll review the history you’ve shared and explore symptoms in more depth.

The visit includes time together as a family and individual conversation.

Part two: making a plan together

At a separate treatment-planning visit lasting approximately 45 minutes, we’ll discuss how your child’s health, experiences, and environment may be contributing to their difficulties. We’ll review the assessment, consider treatment options, and develop a plan together.
